Forms for Advisors
Adult Supervisor Forms
These forms must be signed and filled out by any adults/chaperones/advisors who may be working unsupervised with your members, including the advisor. The documents are attached and include: 1. The Adult Supervisor Code of Conduct 2. The Written Abuse Prevention Policy 3. HOSA District Volunteer Screening and Training Certification Form (one form for all adults is fine) The "Screening and Training Certification Form" is required by our insurance and asks that someone if your building (like an appropriate administrator) signs off that you (and any adult working with HOSA members) has been background checked and has had abuse and molestation prevention training from the district. If you do not have these things at your school, Montana HOSA can help set this up for you.
Conference Forms
Attached are the downloadable Code of Conduct and Medical Release Forms you will need for SLC. Please verify that you have these forms for ALL attendees.
Reporting Forms
Please review the Quality Chapter Document and reporting requirements prior to the start of the school year so that you know what is expected of you. Reports will be collected via a Google Form that is sent out in the spring and due May 19, 2023
